Epub 2013 Mar 13.   <\/a><\/p>\n\n<p><strong>Co-auteurs :<\/strong> L\u00e4dermann A, Lubbeke A, Stern R, <a href=\"https:\/\/cepco.ch\/biography-of-dr-gregory-cunningham\/\" target=\"_blank\" rel=\"noreferrer noopener\">Cunningham G<\/a>, Bellotti V, Gazielly DF.<\/p>\n\n<h2 class=\"wp-block-heading\">R\u00e9sum\u00e9 :<\/h2>\n\n<p><strong>Objectif : <\/strong>L&rsquo;objectif de cette \u00e9tude \u00e9tait d&rsquo;analyser l&rsquo;incidence \u00e0 long terme de l&rsquo;arthropathie de luxation apr\u00e8s une intervention de Latarjet modifi\u00e9e pour l&rsquo;instabilit\u00e9 gl\u00e9no-hum\u00e9rale.<\/p>\n\n<p><strong>M\u00e9thodes : <\/strong>Des informations de suivi \u00e0 long terme ont \u00e9t\u00e9 obtenues \u00e0 partir d&rsquo;une s\u00e9rie cons\u00e9cutive de patients ayant subi une intervention de Latarjet modifi\u00e9e par un chirurgien entre 1986 et 1999. Une analyse de r\u00e9gression multivariable a \u00e9t\u00e9 r\u00e9alis\u00e9e pour examiner la relation entre le d\u00e9veloppement d&rsquo;une arthropathie de luxation et les facteurs li\u00e9s aux patients et \u00e0 la chirurgie. <\/p>\n\n<p><strong>R\u00e9sultats : <\/strong>Il y avait 117 patients (117 \u00e9paules) \u00e0 \u00e9valuer, (35 femmes et 82 hommes) avec un \u00e2ge moyen de 28,4 \u00b1 8,5 (intervalle, 16-55). Le suivi moyen \u00e9tait de 16,2 ans (de 10 \u00e0 22,2 ans). Des signes d&rsquo;arthropathie de luxation ont \u00e9t\u00e9 trouv\u00e9s chez 36 % des patients, class\u00e9s comme Samilson 1 dans 30 %, Samilson 2 dans 3 %, et 3 % Samilson 3 dans 3 % des patients. Les facteurs de risque d&rsquo;arthropathie par luxation comprenaient la chirurgie chez les patients \u00e2g\u00e9s de plus de 40 ans (64,3 contre 34,4 % ; RR ajust\u00e9 2,2, IC 95 % 1,7-2,9) et le positionnement lat\u00e9ral de l&rsquo;apophyse coraco\u00efde transf\u00e9r\u00e9e par rapport au rebord gl\u00e9no\u00efde (82,4 contre 30,4 % ; RR ajust\u00e9 2,3, IC 95 % 1,7-3,2). Les patients pr\u00e9sentant une hyperlaxit\u00e9 ont d\u00e9velopp\u00e9 moins d&rsquo;arthropathie de luxation (15 vs. 42,5 % ; RR ajust\u00e9 0,4, IC 95 % 0,1-0,95).    <\/p>\n\n<p><strong>Conclusion : <\/strong>L&rsquo;apparition d&rsquo;une arthropathie de luxation apr\u00e8s l&rsquo;intervention de Latarjet reste une source d&rsquo;inqui\u00e9tude \u00e0 long terme. Elle est corr\u00e9l\u00e9e \u00e0 la chirurgie apr\u00e8s 40 ans et au transfert lat\u00e9ral de la coraco\u00efde par rapport au rebord gl\u00e9no\u00efdien.
