Elle est le r\u00e9sultat d&rsquo;un m\u00e9canisme cognitif complexe qui comprend l&rsquo;anxi\u00e9t\u00e9, la saillance, la peur et l&rsquo;anticipation. <\/p>\n\n<p><strong>Objectif : <\/strong>L&rsquo;objectif de cette \u00e9tude prospective cas-t\u00e9moins \u00e9tait de corr\u00e9ler cinq scores cliniquement \u00e9tablis \u00e0 l&rsquo;aide de l&rsquo;imagerie par r\u00e9sonance magn\u00e9tique fonctionnelle pour \u00e9valuer les sch\u00e9mas d&rsquo;activation c\u00e9r\u00e9brale chez les patients souffrant d&rsquo;appr\u00e9hension li\u00e9e \u00e0 l&rsquo;instabilit\u00e9 ant\u00e9rieure de l&rsquo;\u00e9paule.<\/p>\n\n<p><strong>M\u00e9thodes : <\/strong>Cette \u00e9tude inclut 28 patients cons\u00e9cutifs masculins droitiers (moyenne \u00b1 SEM, 26,8 \u00b1 1,2 ans) avec un test d&rsquo;appr\u00e9hension de l&rsquo;\u00e9paule positif et 10 participants t\u00e9moins sains appari\u00e9s sans appr\u00e9hension ni ant\u00e9c\u00e9dents d&rsquo;instabilit\u00e9. Les sch\u00e9mas d&rsquo;activation de l&rsquo;imagerie par r\u00e9sonance magn\u00e9tique fonctionnelle li\u00e9s \u00e0 la t\u00e2che et \u00e0 la connectivit\u00e9 fonctionnelle survenant au cours de la stimulation du rep\u00e8re vid\u00e9o de l&rsquo;appr\u00e9hension ont \u00e9t\u00e9 corr\u00e9l\u00e9s \u00e0 cinq tests et scores cliniques : \u00c9chelle visuelle analogique (EVA), score de Rowe pour l&rsquo;instabilit\u00e9, Simple Shoulder Test, Subjective Shoulder Value (SSV) et Western Ontario Shoulder Instability (WOSI). <\/p>\n\n<p><strong>R\u00e9sultats : <\/strong>Les scores de Rowe, de l&rsquo;EVA de la douleur et du WOSI sont en corr\u00e9lation avec le cortex pr\u00e9frontal, le cortex pr\u00e9frontal dorsolat\u00e9ral, le cortex pr\u00e9frontal dorsom\u00e9dial, l&rsquo;aire somatosensorielle et les aires pari\u00e9to-occipitales et temporales (r\u00e9seau du mode par d\u00e9faut). Le score de Rowe est en outre corr\u00e9l\u00e9 avec le p\u00f4le frontal, le cortex midcingul\u00e9 ant\u00e9rieur et les aires visuelles. En outre, le SSV est corr\u00e9l\u00e9 \u00e0 l&rsquo;activit\u00e9 c\u00e9r\u00e9brale li\u00e9e \u00e0 la t\u00e2che dans le gyrus pr\u00e9central bilat\u00e9ral, le gyrus postcentral bilat\u00e9ral et le lobe pari\u00e9tal sup\u00e9rieur bilat\u00e9ral.  <\/p>\n\n<p id=\"block-d2b7639f-462e-49f8-a9d1-384ed339f507\"><strong>Conclusions : <\/strong>Globalement, le score de Rowe fournit le lien le plus fort entre l&rsquo;appr\u00e9hension de l&rsquo;\u00e9paule et les alt\u00e9rations au niveau du cerveau car il est corr\u00e9l\u00e9 avec le plus grand nombre de composantes ind\u00e9pendantes impliquant les zones responsables des fonctions motrices et cognitives, tandis que l&rsquo;EVA de la douleur et le WOSI occupent un lien moyennement fort recrutant moins de r\u00e9seaux c\u00e9r\u00e9braux.
