Le principal crit\u00e8re d&rsquo;\u00e9valuation \u00e9tait l&rsquo;apparition d&rsquo;un syndrome de loge aigu. <\/p>\n\n<p><strong>R\u00e9sultats : <\/strong>Les dossiers de 265 patients (\u00e2ge moyen 48,6 ans) ayant subi 269 fractures intra-articulaires du plateau tibial ont \u00e9t\u00e9 examin\u00e9s r\u00e9trospectivement. Un syndrome du compartiment aigu est survenu dans 28 fractures (10,4 %). Quatre patients pr\u00e9sentaient des fractures bilat\u00e9rales du plateau tibial ; deux d&rsquo;entre eux pr\u00e9sentaient un syndrome de compartiment aigu unilat\u00e9ral, mais aucun ne pr\u00e9sentait un syndrome de compartiment aigu bilat\u00e9ral. Une fracture du tibia non contigu\u00eb ou une luxation du genou et une classification AO\/OTA plus \u00e9lev\u00e9e (type 41-C) ont \u00e9t\u00e9 associ\u00e9es de mani\u00e8re statistiquement significative au d\u00e9veloppement du syndrome du compartiment aigu dans l&rsquo;analyse de r\u00e9gression multivariable, tandis qu&rsquo;un \u00e2ge plus jeune (&lt;45 ans), le sexe masculin, un grade de Schatzker plus \u00e9lev\u00e9 (IV-V-VI), un rapport d&rsquo;\u00e9largissement tibial plus \u00e9lev\u00e9 (\u22651,05) et un rapport de d\u00e9placement f\u00e9moral plus \u00e9lev\u00e9 (\u22650,08) ont \u00e9t\u00e9 associ\u00e9s de mani\u00e8re significative dans l&rsquo;analyse ajust\u00e9e \u00e0 l&rsquo;\u00e2ge et au sexe.   <\/p>\n\n<p><strong>Conclusions : <\/strong>Deux param\u00e8tres li\u00e9s \u00e0 la survenue d&rsquo;un SCA dans les fractures du plateau tibial ont \u00e9t\u00e9 mis en \u00e9vidence dans cette \u00e9tude : la pr\u00e9sence d&rsquo;une fracture du tibia non contigu\u00eb ou d&rsquo;une luxation du genou, et une classification AO\/OTA plus \u00e9lev\u00e9e. Ils peuvent \u00eatre particuli\u00e8rement utiles lorsque les r\u00e9sultats cliniques sont difficiles \u00e0 \u00e9valuer (signes cliniques douteux, patients obtus, s\u00e9dat\u00e9s ou intub\u00e9s) et doivent \u00e9veiller les soup\u00e7ons du chirurgien traitant. Dans ces cas, des examens cliniques r\u00e9guliers et\/ou des mesures de la pression intra-compartimentale doivent \u00eatre effectu\u00e9s avant et apr\u00e8s l&rsquo;intervention chirurgicale, m\u00eame si le syndrome du compartiment aigu semblait peu probable lors de l&rsquo;\u00e9valuation initiale.
