Cependant, le r\u00f4le de la grosse tub\u00e9rosit\u00e9 (GT) devrait \u00eatre tout aussi important. L&rsquo;objectif de cette \u00e9tude \u00e9tait de proposer un nouveau marqueur radiographique, l&rsquo;angle GT (GTA), qui mesure la position du GT par rapport au centre de rotation de la t\u00eate hum\u00e9rale. L&rsquo;hypoth\u00e8se \u00e9tait qu&rsquo;une valeur d&rsquo;angle plus \u00e9lev\u00e9e serait associ\u00e9e \u00e0 une plus grande probabilit\u00e9 de d\u00e9tecter une rupture de la coiffe des rotateurs.   <\/p>\n\n<p><strong>M\u00e9thodes : <\/strong>Pendant un an, des patients ont \u00e9t\u00e9 recrut\u00e9s prospectivement dans une clinique sp\u00e9cialis\u00e9e dans l&rsquo;\u00e9paule d&rsquo;un seul \u00e9tablissement et r\u00e9partis en deux groupes diff\u00e9rents. Le groupe de patients \u00e9tait compos\u00e9 de personnes pr\u00e9sentant une rupture d\u00e9g\u00e9n\u00e9rative de la coiffe des rotateurs impliquant au moins le supra-\u00e9pineux. Le groupe t\u00e9moin \u00e9tait compos\u00e9 de personnes ne pr\u00e9sentant pas de pathologie de la coiffe des rotateurs. Les individus des deux groupes pr\u00e9sentant des alt\u00e9rations cong\u00e9nitales, post-traumatiques ou d\u00e9g\u00e9n\u00e9ratives de l&rsquo;hum\u00e9rus proximal ont \u00e9t\u00e9 exclus. Le GTA a \u00e9t\u00e9 mesur\u00e9 sur une radiographie ant\u00e9ropost\u00e9rieure de l&rsquo;\u00e9paule, le bras \u00e9tant en rotation neutre, par 3 observateurs \u00e0 2 moments diff\u00e9rents.    <\/p>\n\n<p><strong>R\u00e9sultats : <\/strong>L&rsquo;\u00e9tude a recrut\u00e9 71 patients (33 patients, 38 t\u00e9moins). La valeur moyenne de la GTA \u00e9tait de 72,5\u00b0 (intervalle, 67,6\u00b0-79,2\u00b0) chez les patients et de 65,2\u00b0 (intervalle, 55,8\u00b0-70,5\u00b0) chez les t\u00e9moins (P &lt;.001). Une valeur sup\u00e9rieure \u00e0 70\u00b0 multipliait par 93 les chances de d\u00e9tecter une rupture de la coiffe des rotateurs (P &lt;.001). La fiabilit\u00e9 inter-observateur et intra-observateur \u00e9tait \u00e9lev\u00e9e.   <\/p>\n\n<p><strong>Conclusions : <\/strong>La morphologie de la GT est impliqu\u00e9e dans les ruptures de la coiffe des rotateurs.
