L&rsquo;angle critique de l&rsquo;\u00e9paule (CSA) et l&rsquo;angle du trochiter (GTA) sont des param\u00e8tres de mesure bidimensionnels qui ont \u00e9t\u00e9 valid\u00e9s pour l&rsquo;\u00e9valuer radiologiquement. Ces marqueurs sont toutefois statiques et ne tiennent pas compte de l&rsquo;effet dynamique du mouvement gl\u00e9no-hum\u00e9ral.  <\/p>\n\n<p><strong>Objectifs : <\/strong>Cette \u00e9tude visait \u00e0 mieux comprendre la biom\u00e9canique du conflit sous-acromial gr\u00e2ce \u00e0 une simulation dynamique bas\u00e9e sur un mod\u00e8le biom\u00e9canique 3D valid\u00e9 associant la cin\u00e9matique articulaire et la tomodensitom\u00e9trie reconstruite en 3D.<\/p>\n\n<p><strong>Conception et m\u00e9thodes de l&rsquo;\u00e9tude : <\/strong>Soixante et un patients ont \u00e9t\u00e9 inclus dans cette \u00e9tude : un groupe de cas de 44 patients pr\u00e9sentant des ruptures d\u00e9g\u00e9n\u00e9ratives de la coiffe des rotateurs impliquant uniquement le supra-\u00e9pineux, et un groupe t\u00e9moin de 17 patients ne pr\u00e9sentant pas de rupture de la coiffe des rotateurs. Les patients ayant subi des interventions chirurgicales ant\u00e9rieures, des ruptures traumatiques de la coiffe et une arthropathie de rupture de la coiffe ont \u00e9t\u00e9 exclus. La CSA, la GTA et l&rsquo;amplitude de mouvement sans conflit (IF-ROM) de l&rsquo;articulation gl\u00e9no-hum\u00e9rale en scaption ont \u00e9t\u00e9 calcul\u00e9es. Des tests de corr\u00e9lation ont \u00e9t\u00e9 utilis\u00e9s pour d\u00e9terminer la relation entre l&rsquo;amplitude de mouvement et la CSA, la GTA et les valeurs combin\u00e9es de la CSA et de la GTA.   <\/p>\n\n<p><strong>R\u00e9sultats : <\/strong>La CSA et la GTA \u00e9taient significativement plus \u00e9lev\u00e9es dans le groupe des ruptures de la coiffe des rotateurs (p = 0,001 et &lt; 0,001), tandis que l&rsquo;IF-ROM \u00e9tait significativement plus \u00e9lev\u00e9 dans le groupe t\u00e9moin (p = 0,001). Il n&rsquo;y avait pas de corr\u00e9lation globale entre la CSA et la GTA (R = 0,02, p = 0,8). La corr\u00e9lation individuelle entre les deux angles et l&rsquo;IF-ROM \u00e9tait n\u00e9gativement faible pour la CSA (R = -0,4, p &lt; 0,001) et n\u00e9gativement mod\u00e9r\u00e9e pour la GTA et l&rsquo;IF-ROM (R = -0,5, p &lt; 0,001). Toutefois, la combinaison des deux angles a donn\u00e9 lieu \u00e0 une corr\u00e9lation n\u00e9gative \u00e9lev\u00e9e avec l&rsquo;IF-ROM (R = -0,7, p &lt; 0,001).   <\/p>\n\n<p><strong>Conclusion : <\/strong>Le r\u00e9tr\u00e9cissement de l&rsquo;espace sous-acromial pendant la scaption est fortement corr\u00e9l\u00e9 aux valeurs cumul\u00e9es de l&rsquo;ATG et de la CSA.
